It has a new name, but I don't know it for sure. Ask your international office to issue you a new one (three copies) and most International office will provide a letter too. And your dependant's passport. To be safe, include your finacial support evidence, such as your bank statement. It is not hard for J2 applicants to get J2 visas. Good luck.
