question about I-140 and I-134

I am applying EB-1 (OR outstanding researcher):

1. in I-140, page 2, part 5. additional information about the petitioner, because I am applying eb-1 OR, the petitioner should be company, right? In 2. If a company, give the following:------
DOL/ETA case number____. What is that DOL/ETA case number mean? Where can I find it?

2. In my wife's 485 application, I need to fill I-134, Affidavit of support for her, right?
in the first qestion,"If you are not a native born United States citizen, answer the following as appropriate:a,b,c,d.", but I cannot choose any one from abcd, because I amnot citizen and donot have permanent resident, how can answer this question?
