What's the ARC Sworn Statement

I will be for a naturalization oath ceremony next month. But I lost (misplaced) my green card. According to USINS instructions, I have to bring my lost ARC sworn statement with photo ID. Can anyone tell me what the ARC Sworn Statement is and where I can get it? Thanks
