Answer to Q1:
Your hu*****and should check "h" and indicate "concurrently file I140 and I485" there. Of course, you should check "b".

Answer to Q2:
I used the Black Control Numbers when filing the I-485s of mine and my family's. This way worked well for us. But some people believe the correct option should be the RED #.

BTW, I concurrently filed the I-140s (EB1a + NIW) and I-485s at the end of April, 2004, and our cases were all approved by early May, 2005. -- Done by myself -- NSC.
