visa/I-84 needed for H4?

Hello, can someone with H4 experience tell me what docs are required for H4 visa application in China? I will apply for a new H1 from INS. Is I-797 approval notice enough? Do I also need to leave US and get a H1B visa on my passport? And I-94 card with my new H1B status? Thank you.
