First time changing Hard Drive. Pls Help.

Situation:
System very slow. Everytime turn on computer, Disk Check will be performed, and there are many bad areas.

Questions:
1. Do I have to change Hard Drive? I have used computer for a long time, but this is the 1st time to change Hard Drive.

2. If Yes, how should I change Hard Drive?

3. How should I copy between old Hard Drive and new Hard Drive?
