Looks to me he is a good hands on person but lacks the motivations to be a leader such as having a vision to execute a project from end to end, identify and mitigate risks.
If he doesn't improve, is demoting him to be hands on an option? If you have to print out his improvement plans, you might as well send him into PIP with HR.
