A couple things you can do.

1. Immediately document the one on one conversations with dates
2. Check with her former bosses and see if they think highly of her and her prior performance
You will need #1 if you want to put her on probation before getting rid of her. #2 is the homework you want to do before taking it to HR or figure out some other way. Don't think talking to her directly is going to help.
